Step 2 - Before going to an agency to ask for assistance, gather the required paperwork.

Agencies may ask for:

  • Driver's License or State Picture ID

  • Social Security card or number for you and other family members

  • Birth Certificates for your children

  • Your last pay stub

  • Medicaid or insurance card, for medical or dental help

  • Utility shut-off notice or eviction notice for help with these

  • Decision notice from DHS

Step 4 - Before going to an agency, call first

Step 5 - When going to an agency, go early. Going to an agency too close to the lunch hour or the end of the business day won't give enough time to process applications. This may delay getting help.
